Subproject 1
In the first subproject, patients with elevated creatinine levels are prospectively recruited and recorded in GP practices cooperating with the university hospital Greifswald. The aim is to identify a total of 500-700 patients with chronic renal insufficiency and to examine and evaluate their care in the last 12 months.

Subproject 2
In a further sub-project, the interface from the general practitioner to the nephrologist is discussed with guiding questions in three focus groups with 6 to 8 GPs each.

Subproject 3
The care of patients with chronic renal insufficiency is to be examined longitudinally. For this purpose, the 10-years (2002-2013) of statutory health insurance billing data and the laboratory medical examinations from the population-based cohort study SHIP (Study of Health in Pomerania) are evaluated. In total, data from approx. 4000 patients are available, of whom approx. 10% have renal dysfunction.
Subproject 4
The last subproject focussed on writing a guideline for general practitioners (S3 guideline of the professional societies DGfN and DEGAM) for the long-term care of patients with chronic renal insufficiency and a set of quality indicators to measure the quality of outpatient care of patients with chronic renal insufficiency not requiring dialysis.
