Child and Adolescent Epilepsy

Epileptic seizures occur in approximately 1% of children and adolescents, and are thus one of the most common neurological disorders in this age group. Cerebral defects such as premature birth, perinatal asphyxia, cranial brain trauma, brain tumours, or brain malformations can be causes of symptomatic epilepsies. There are also idiopathic epilepsies, for example, due to mutations of ion channels or genes in the area of neurotransmitters.

Treatment / Therapy

  • Usually medication
  • Aufdosierungsphase von meist 2-4 Wochen mit angestrebter Therapiedauer von zunächst 3 Jahren, anschließend Ausschleichen über 1 Jahr 
  • A dosage phase of usually 2 to 4 weeks, with a desired initial treatment duration of 3 years, then tapering off for 1 year
  • Implementation of monotherapy, depending on effectiveness with one or two medications administered in succession (applies to approximately 1/3 of children)
  • Implementation of a double or triple combination (another third of the children)
  • There are problematic cases in 10-30% of children, with either insufficient seizure control or intolerable side effects
    • In case of lesional epilepsies there is a surgical therapy option, if the epileptogenic focus is located in the non-essential brain area, and can be unambiguously identified and removed without a neurological deficit
  • For newly occurring epilepsy, generally inpatient treatment with diagnostics and medication over 1-2 weeks
    • Then two to four outpatient visits per year with EEG and laboratory checks

The department of neuropaediatrics and metabolic diseases is the paediatric section of the Greifswald epilepsy centre. It is certified seizure outpatient department of the League Against Epilepsy. Its head has the certificate for presurgical epilepsy diagnostics.
